Add Phase 3 to ship-it: after commit+PR are published (Phase 2), check the
actual persisted commit message(s) and PR description for AI attribution
footers injected by hooks outside the visible conversation, and strip them
via git commit --amend + force-push and gh pr edit. Only runs when a
global/user/project rule forbids attribution footers.
Sync README docs: ship-it now documented as three phases; mirror-it now
documents the GitHub target-repo existence check/creation step (already
shipped in plugins/dist via #9, but undocumented in the README).
Test plan
/ship-it on a repo with a no-attribution rule: confirm Phase 3 checks
the real commit/PR after Phase 2 and strips any sneaked-in attribution.
/ship-it on a repo with no such rule: confirm Phase 3 is skipped.
Confirm mirror-it and ship-it README sections match current SKILL.md
behavior.

Hooks outside the visible conversation can inject Co-Authored-By or
"Generated with Claude Code" lines into a commit or PR after it's
already created, even when a no-attribution rule is active. Add Phase
3 to ship-it: after Phase 2 publishes, check the actual persisted
commit message(s) and PR description (not the text composed inline)
for attribution, and strip it via commit --amend + force-push and gh
pr edit. Only runs when a global/user/project rule forbids attribution
footers.
Also syncs README docs for ship-it (now three phases) and mirror-it
(GitHub target-repo existence check/creation, already shipped in
plugins/dist but undocumented).
